To request patient records, please click the button.  Upon receiving your request for records, we will contact you to obtain signed medical consent to release records and begin working on your request.

Per federal law, we are allowed up to 30 days to release records.  If records are needed sooner than this, then consider requesting expeditated records.  Rates for such requests are located in the link to the right.

We do not have records from JP Interventions and cannot assist you in obtaining them.

If you are a social worker, CASA advocate, attorney, or collateral personnel consider instead requesting a "Progress Update" if this is suitable for your needs.  A progress update will provide current services, diagnosis and general impressions of the client and family and can be released in as little as 3 days.  These are also provided at no cost.
